Qantas Flight Disruptions Claim Form

You may use this form, if:

• you are affected by the current flight disruption;
• have made a booking directly with Qantas (if you have made a booking via a Travel Agent, you will have to contact your Travel Agent for any changes)
• you are eligible to change your flight with no fees, as detailed in the fare waiver
• you hold a valid Qantas (081) tickets for a flight that has been disrupted by the current flight disruption.
• the fare changed to, is within the same cabin, eg. Economy to Economy
